Information for Professionals

Keighley Healthy Living work with local community partnerships, PCN’s, local authorities and other health and community organisations and charities to deliver accessible, inclusive health initiatives that are responsive and representative of local health needs.

We act as a community anchor and driver to enable cohesive health care pathways which connect people’s clinical and community care. KHL has been a bridge organisation, exemplifying how well-executed partnership working provides benefits for individuals and cost savings for institutions such as the NHS.

Whilst day to day delivery of our services is the key priority, our underpinning objective of achieving system change is also at the forefront of our ambition as an organisation.

We understand the importance of collaborative working and measuring impact to achieve the best possible outcomes for our communities.

Referring into KHL’s services

Healthy Communities

Users can self-refer into this project by booking a session or group listed on the Activity Calendar – call 01535 677177 or email bookings@khl.org.uk to make a booking.

Maternity, Young Parents & Families

Maternity Circles groups are run on a self-referral basis. There is no need for attendees to book – just drop in.

T-Natal is accessed by referral only, due to the 1-1 support this project offers. The referral form can be accessed here:

Rethinking Pain

Users of this service must be referred by their GP after completing a Pain Support Plan together. The form must be submitted through SystmOne. 

Mental Health Connect

Self-referrals and referrals on behalf of others living in BD20, BD21, BD22 and LS29 postcode areas are accepted via this specific referral form:

Blood Pressure Connect

Patients of Modality and WACA primary care networks with raised blood pressure are eligible to be referred into this project via their GP.

MAST

Hospital staff at Bradford Royal Infirmary and Airedale General Hospital are able to refer to this service.

Pathways

Over 18s who need support can contact Project 6 by calling 01535 610180.

Healthy Holidays

These sessions are run during the school holidays and are offered to children who receive free school meals in the Keighley area. To find out more, call 01535 677177.

Our doors are not only open to current and new service users. We welcome visits from local businesses, VCS organisations and health care professionals. If you would like to request a visit, we can have the appropriate staff member meet you and give you a guided tour.
