Blood Pressure Connect

Blood Pressure Connect is a free, local opportunity to support people who have been informed by their Doctor (GP) or a health professional that their blood pressure is higher than the normal range. Once referred, KHL will support you to make changes to help lower your blood pressure and reduce risk of future illness or need for lifelong blood pressure medication.

The below figures are from 23/24:

72% saw a reduction in blood pressure readings

32% saw a reduction in BMI

89% made at least one lifestyle change

Here are some small changes that people have made during their time with the programme:

Swapping to decaf drinks

Implementing a sleep routine

Reducing the amount of salt added to food

Increasing water intake

Accessing support for mental health

Implementing 5 alcohol free evenings

Measuring blood pressure at home

Decreasing time sitting down

The Blood Pressure Connect approach is not ‘one size fits all’ in supporting you to reduce your blood pressure and improve your health and wellbeing. Our team believe that if you have some extra support and time, you are more likely to succeed, by getting involved in this new joined up approach to reducing your blood pressure.

When you join the service, you’ll be assigned a dedicated health coach who will support you in making positive lifestyle changes to help lower your blood pressure.

During your appointments, you will receive:

  • Clear information to help you understand hypertension and how lifestyle choices affect it
  • One-to-one health coaching to set realistic and achievable lifestyle goals
  • Guidance and signposting to local activities and services that support your goals
  • A free blood pressure monitor to help you track your progress at home
